The period covered by the annual performance evaluation forms this year is April 1, 2017 to March 31, 2018. The timeline for this year’s annual submission is no later than the dates listed below for each step:

  • March 31, 2018 – Performance evaluation period closes
  • April 6, 2018 – Self-Appraisal Form to be submitted to supervisor
  • May 4, 2018 – Formal Performance Evaluation Form to be signed by employee
  • June 1, 2018 – Signed Formal Performance Evaluation Form to be given to Human Resources
  • June 30, 2018 – Human Resources to report supervisor compliance with process to Deans/VPs
