The period covered by the annual performance evaluation forms* this year is April 1, 2018 to April 30, 2019. The timeline for this year’s annual submission is no later than the dates listed below for each step:

  • April 30, 2019 – Performance evaluation period closes
  • May 10, 2019 – Self-Appraisal Form to be submitted to supervisor
  • June 21, 2019 – Formal Performance Evaluation Form to be signed by employee
  • July 5, 2019 – Signed Formal Performance Evaluation Form to be given to Human Resources
  • July 31, 2019 – Human Resources to report supervisor compliance with process to Deans/VPs
