Eligible Dependents and Verification

Eligible Dependents

Employees who are appointed to work at or above a 75% full-time equivalency basis are eligible to enroll their dependents in select benefits with Wright State University. Your enrolled dependents must meet the following eligibility definitions, and you will need to provide dependent verification as noted below:

  • Spouse, an individual married to a benefit-eligible employee and is supported by an active government-issued marriage certificate.
  • Child up to the age of 26, who is a biological child, step-child, legally adopted child, or a child for which the employee or employee’s spouse has permanent legal custody or legal guardianship, or for any child covered by a Qualified Medical Child Support Order.
  • A child, as described above, but is age 26 or older and is certified as disabled, unmarried, and dependent on you for financial support. 


Dependent Verification

If you are enrolling your spouse and/or dependents in one or more of the following benefits, you will be required to provide supporting documentation as presented below:

  • Medical
  • Dental
  • Vision
  • Tuition Remission
  • Dependent (Spouse/Child) Life Insurance

Supporting documentation for the enrolled dependent relationship will be required at the time of hire, qualifying events, or dependents enrolled during the annual Open Enrollment. In addition, dependents verification audits could be implemented periodically as deemed necessary by the University. 

Spouse

To enroll a spouse, you will need to provide the following two documents:

  • A government-issued marriage certificate, and
  • A joint document addressed to both parties and dated within the last 90 days.
    • Examples: utility bills, mortgage statements, auto insurance, bank statements, etc.

Dependent up to Age 26

To enroll a dependent up to age 26, you will need to provide the following documents: 

  • Biological or Adopted Child: Birth certificate or court document (paternity test, divorce decree, or a Qualified Medical Child Support Order)
  • Stepchild: Birth certificate or court document (paternity test or divorce decree) and confirm eligibility of the spouse (as noted in spouse requirements above)
  • Legal Guardianship or Legal Custody: Court document assigning a minor child to an employee under permanent legal guardianship or legal custody
  • Foster Child: Agency assignment or court document grating child to an employee under foster care

Dependent with a Disability Over Age 26

To enroll a dependent with a disability over age 26, you will need to complete a "Dependent with a Disability Certification" form: 

  • You will be certifying the dependent is incapable of self-sustaining employment because of a physically or mentally disabling injury, illness or condition, an
  • You will be certifying the dependent relies on you for financial support and maintenance due to the physically or mentally disabling injury, illness or condition.
