About the Health Management Initiative

Wright State University Human Resources continues to provide a Health Management Initiative (HMI) for employees who elect coverage on our medical plans. The purpose of this initiative is to raise awareness of behaviors that impact health, increase the detection of chronic conditions like cardiovascular disease and diabetes, and encourage participants to begin and/or continue to make healthier lifestyle choices.

As an example of the intent of this initiative, based on the aggregate data from 2024-2025, a total of 999 participants completed a biometric screening. The aggregate screening results identified the following:

  • High cholesterol levels: 8%
    Hypertension 2 or crisis levels of blood pressure: 9.5%
  • High A1C and glucose results or diabetic: 7%
  • Obese or severely obese: 35%

The above aggregate data will be used to inform programming that might be beneficial to the university at large and will be compared to future aggregate data to determine the changes in the health of our employee population.


Highlighted Program Changes for 2026

The following changes are being made to this year’s program:

  • If you elect not to complete the Health Management Initiative actions by August 31, 2026, a $160 non-participation fee will be assessed to your monthly premiums beginning January 1, 2027. The non-participation fee will apply to all employees enrolled in a Wright State medical plan (as the policyholder) who do not meet the requirements of the Health Management Initiative.
  • We’ve increased the number of on-site biometric screening events at the Dayton campus. You may still choose to have your biometric screening performed by your Primary Care Physician (PCP) or any LabCorp location as long as the respective paperwork is completed and submitted by the deadline. Details about the on-site biometric screenings can be found below.


Program Requirements & Eligibility

In order to avoid being charged a non-participation fee in 2027, employees enrolled in a Wright State medical plan (as the subscriber) should complete the following actions between November 1, 2025 and August 31, 2026:

  • Complete a confidential online Health Risk Assessment (HRA) through HealthWorks,
  • Participate in a biometric screening using one of the three options detailed below and
  • Visit their Primary Care Physician (PCP) to discuss the biometric screening results and submit the Offsite Screening Results Form to HealthWorks.

Non-Participation Fee

If all requirements are not completed by August 31, 2026, a $150 non-participation fee will be assessed beginning January 1, 2027. The non-participation fee will apply to all employees enrolled in a Wright State medical plan (as the subscriber) who do not meet the requirements of the Health Management Initiative.


Health Risk Assessment (HRA) Details

The HRA is a confidential, self-administered survey that asks a series of questions about your lifestyle habits and health history and only takes 10 minutes to complete on the HealthWorks site. It can be completed 24/7 and is an important first step to improving your health as the results may indicate where changes can be made to avoid future health care issues.


Biometric Screening (Blood Draw) Details

Biometric screenings provide a clinical assessment of key health measures. These results may reveal an increased risk for health conditions, such as diabetes and heart disease. This may take about 10 minutes to complete at our on-site screenings and is an important part of creating a healthier you. Making changes based on these results is also a way to save on your future health care expenses. The HealthWorks website will allow you to upload your screening information from a Labcorp or PCP visit 24/7. At least 20 on-campus screening events will be offered at the Dayton Campus with at least one at the Lake Campus as well. 

Biometric screenings include:

  • Blood chemistry profile and complete blood count
  • 27 individual tests, including cholesterol, A1C, glucose, kidney and liver enzymes, and additional tests to detect anemia, infection, and more
  • Blood pressure
  • Body mass index (BMI)
  • Waist circumference

Screening tips:

  • Results are most accurate with an 8–10 hour fast before your appointment.
  • You may drink black coffee or tea (no cream or sugar), and water is encouraged.
  • Take all medications as prescribed by your doctor.

Additional Testing Available for Purchase

Additional tests are available for purchase with your on-site biometric screening. Payable by exact cash, check, Visa, Mastercard, HSA, or FSA debit card. Insurance reimbursement for additional testing is subject to your plan and not guaranteed.

Biometric Screening Options and On-Campus Campus Screening Dates

To complete your biometric screening requirement, there are three options:

  1. Schedule an on-site biometric screening on the Dayton Campus, Lake Campus, or STEM Campus, as listed in the next section
  2. Download and print the Going to Labcorp Form and take it to any Labcorp location. Once completed, LabCorp will fax the form to HealthWorks.
  3. Download and print the Offsite Screening Results Form and take it to your PCP. Once the form is completed, your PCP can fax it to the number on the form. Please retain a copy for your records.

Program Instructions and Checklist

To continue your journey to a healthier you, and to avoid being charged the non-participation fee, complete the easy steps below by August 31, 2026. 

It is highly recommended to schedule an appointment with your PCP early to ensure availability and meet the August 31, 2026 deadline.

If you haven’t already, establish a HealthWorks Online Account (all new users must create an account)

  1. Visit www.myHealthWorksportal.com
  2. Click ‘sign up now’ and create an account
    1. New user’s will be required to create an account. The Sign-Up process requires you to click ‘sign up now’ and follow prompts (input your name as it appears on your Anthem insurance ID card).  Once you register your email, it becomes your username. Wright State’s Access Code: FY7A9

Complete your Health Risk Assessment (HRA)

  1. After you have completed step 1, click the My Wellness tab.
  2. Click on Health Risk Assessment in the navigation bar.
  3. Complete the assessment.
  4. Be sure to click Save when all sections are completed.

Schedule and Attend Biometric Screening Appointment

A biometric health screening can be completed in one of the following ways:


OPTION 1

On-Site Screenings—Dayton and Lake Campuses, STEM School

  • After you have completed the registration process in step 1, click on the My Appointments tab.
  • Click on Onsite Screening.
  • Select a date and time.

If you complete an on-campus screening, you will automatically receive credit for the verified biometric screening and do not need to upload further information. The number of available on-campus screening events are limited.

OPTION 2

Primary Care Physician (PCP)

If you choose to have your biometric screening completed by your PCP, you will need to download and print the Offsite Screening Results Form. Please have your physician complete section 2 titled “Patient Results” and fax it to HealthWorks at the number listed on the form. Please retain a copy of the form for your records.  View instructions for this option.

 

OPTION 3

Labcorp

You can schedule a screening appointment at the Labcorp location of your choice. Take a copy of the “Going to LabCorp form” with you and have it completed at the time of your appointment. View instructions for this option. You must take the HealthWorks “Going to LabCorp” form to your appointment. Once completed, LabCorp will send the results to HealthWorks.

View Your Results

Once you have completed the Health Risk Assessment (HRA) and received your biometric screening, log in to myHealthWorksportal.com (within 3 to 5 business days of completing your on-site screening or uploading your off-site results) to view your Health Risk Report. The report combines the results from your HRA and biometric screening and provides you with results and recommendations about making healthier lifestyle choices.

You can view and print your Biometric Screening Results online. No identifiable information is shared with Wright State University. As noted in the privacy section below, the university only receives aggregate data that cannot be tied to a particular individual.

Steps to view your results:

  1. Visit www.myHealthWorksportal.com
  2. Click on Login.
  3. Enter your username and password
  4. Click on the My Wellness tab.

Click on Biometric Screening Results and review your report.

Click on Health Risk Report and review your report.

Primary Care Physician (PCP) Visit

If you completed your biometric screening on-site or at LabCorp, schedule an annual routine wellness visit with your PCP. Take your screening results and the HealthWorks’ Offsite Screening Results Form to your appointment to review your results and discuss your overall health and applicable preventative cancer and other screenings that might be appropriate for you. Your physician must complete section 3 of the Offsite Screening Results Form.

The Offsite Screening Results Form must be signed and submitted to HealthWorks via one of the following methods:

If completing your biometric screening with your PCP, your physician must complete only section 2 of the Offsite Screening Results Form.

 

Additional information regarding PCP Visits

  • Each calendar year, Wright State's medical plans cover one annual routine wellness visit with a PCP at no cost to you.
  • If you received an annual visit in 2025, you should consider scheduling your appointment after January 1, 2026, to avoid out-of-pocket expenses. Any annual PCP visits and/or biometric screening performed after August 31, 2025, will be accepted if your physician completes the required documentation.
  • Need a PCP? Anthem can assist you with finding an in-network provider. During registration, you will be prompted to provide your subscriber ID number on your Anthem insurance card. Anthem’s Sydney Health App allows you to manage your healthcare from your phone or computer. Check claims: See what's covered and what you owe. Get your ID card: Share, fax, or email your ID Card from your device. Find a Doctor: Look for doctors in your plan. Get Medicine: Refill your prescriptions online. Estimate costs: Compare costs and quality for common procedures. Download the Sydney Health app at the Apple Store or Google Play.


Privacy and HealthWorks' IT Practices and Procedures

HealthWorks, Wright State University's biometric screening vendor, will administer the Health Risk Assessments and biometric screenings and collect the Primary Care Physician Verification forms. The privacy of participants' information is strictly protected! The university receives only aggregated data for the participating population and will never know individual assessment answers or biometric screening results.

  • Go to www.myhealthworksportal.com & login
  • Go to your personal dashboard
  • Click the Forms link in the navigation bar
  • Click All Forms
  • Download and print the desired form to go to either LabCorp or your doctor’s office (your program may offer one or both of these options)
  • Don’t forget to take your form with you to your appointment!
