Service Requests

  • Support and Project Request (formerly system modification request) 
    Submit support and project requests to Information Services here. This includes issues in production (live), reporting a bug, new feature/project/report requests(requires authorization with campus username and password).
  • Telecommunications Service Request (PDF) 
    Request telecommunications services, such as phone, voicemail, and network drops.
  • VTS Services Request 
    Request video services through the Video Technology Services department.

Classroom Support

  • Classroom Equipment Instruction Request Form​  
    Request individual classroom equipment instruction.

  • Classroom Equipment Preferences Form 
    As a Faculty/Staff member, submit a request for your preferred equipment to be accessible from a specific classroom.
  • Classroom Problems and Requests 
    Use this form to report problems to an electronic or computer classroom. The information on this form will be reported to the appropriate department via email. If the problem you are having requires IMMEDIATE assistance, please do not use this form. Instead, contact the CaTS Help Desk at (937) 775-4827 for assistance.
  • Equipment Request Form (PDF) 
    Request a specific piece of equipment to be used for educational purposes.

Network Access

  • Faculty Backup Drive Request (Faculty Members Only
    All current faculty members, deans, and chairs may request an external USB hard drive for the primary purpose of backing up their university-owned computer system.
  • VPN Registration/VPN Request Form 
    Request access to the Wright State Virtual Private Network (VPN).


  • Appearance Release (PDF) 
    Grant Wright State permission to use your name and/or one or more portraits, pictures, photographs, video and audio recordings of you ("Appearances"), or reproductions or derivatives of the same, in any form of education, communication, and/or promotion purposes, unless otherwise noted.
  • Comments/Suggestions 
    Submit a question, comment or concern to the CaTS Help Desk.
  • Exception Request 
    Request an exception from a current CaTS policy in order to accommodate a technological need or request that falls outside of the policies set forth by CaTS.
  • Faculty/Staff Listserv Subscriptions 
    Use this page to subscribe and unsubscribe from the university email lists.
  • Non-Supported Product Form (PDF) 
    If you prefer to purchase a product not supported by CaTS, fill out this form acknowledging your understanding of the responsibilities that come along with using this non-supported product (more information through link).
