Release of Information

The Office of Financial Aid has received a request to release information.  In order for us to review or complete your request, you must complete the Financial Aid Release of Information Request Form (DocuSign).  Please note: not all requests can be accommodated due to privacy laws that protect your student information.  Please review the Release of Information Policy available on our website.

DocuSign will allow you to electronically complete, sign, and submit the form online, as well as attach any requested documentation. If you complete the form using DocuSign, the Office of Financial Aid will automatically receive your completed form and any attached documentation. WINGS will be updated within 1-3 business days. For step by step instructions on how to use DocuSign, please review some helpful DocuSign Instructions.
