  • Catering Request Waiver Form
    Wright State University policy (5301.4(c)(1)) requires the use of Wright State Hospitality Services for any food in public university spaces or if purchased with university funds.  Use this form to request an exemption from that policy.
  • Gift-in-Kind Form (PDF)
    This form is to be completed and signed by the sponsor/donor and submitted with the Catering Request Waiver Form.
  • Event Services and Youth Programs Event Request Form (PDF)
    Use this form to request event space on the Wright State University campus.  You can fill out and submit the form electronically.  Please carefully follow the instructions that appear when you click the “Send This Form Via Email ” button on the form.  If you use a webmail application (such as WINGS, Gmail, Yahoo, Hotmail, AOL, etc.), you must first save the completed form to your computer and then manually attach it to your email. You may also fill out (or partially fill out) the form and save it to your computer.  You can then complete or update your information at a future date, re-save the form with the updated information, and attach it to an email sent to event_services@wright.edu.
  • Video Technology Services - Campus Digital Signage
    The Student Union has placed digital flat screen monitors throughout the building that will serve as virtual bulletin boards for the Wright State community.  Due to staff reductions, content is currently being managed by Video Technology Services.  To submit events for posting, please click the link above and then submit their Digital Signage Request form.
  • SUAO Large Poster Guidelines & Form (DOCX)
    The Wright State University Student Union Administrative Office has assumed the task of distributing posters across campus. Please review the attached guidelines before ordering posters. Please print out and complete the included form, and submit it to event_services@wright.edu or drop it off at the Student Union Administrative Office front desk in 023 Student Union.
  • Agreement to Sell Tickets (PDF)
    Student organizations, university departments, and Miami Valley area entertainment facilities that want to utilize our services to sell tickets for their events through our Box Office must complete this form.

  • Meet Set-Up Form (PDF)
    Clients who have already reserved the Wright State University Natatorium must complete this form and submit it to aquatics@wright.edu prior to any swim meet.   • Demonstrations and Marches Form (PDF)
    In accordance with University Policy 1260.04 Operational Procedures for Demonstrations and Marches, a registration form must be submitted and approved not less than twenty-four (24) hours prior to the planned activity.  Please return the registration form to the Office of the Vice President for Student Affairs, 362 University Hall.
