Application

The Student Union & Event Services and Campus Recreation scholarship application is composed of five parts:

1. Typed application/nomination form - [ Download (pdf) ] or [ Download (doc) ]
2. Typed essay - detailing contributions to the Student Union and future aspirations (one typed page, no more than 1000 words).
3. Typed cover letter and resume
4. Two recommendations - one must be completed by your immediate supervisor and one by someone who knows your work. Applicant must provide form and envelope. [ Download (pdf) ]
5. Optional peer nominations - must complete same forms as applicant, excluding resume and cover letter. Applicant responsible for providing photocopies of forms to his/her peer. [ Download (pdf) ]

After printing and completing this form make sure to keep a copy and then submit it. Make sure to send your typed essay, resume and cover letter in a sealed envelope to the Scholarship Selection Committee, c/o Pam Davis, WSU Student Union, 186 SU, 3640 Colonel Glenn Highway, Dayton, OH, 45435.
