The application for services from the Office of Disability Services is separate from the application for admission to the university. Students are encouraged to contact Undergraduate Admissions to apply for admission to the university prior to applying for services with Disability Services.

    • If the electronic forms are inaccessible, please contact the Office of Disability Services at 937.775.5680 to obtain a printed copy of the application materials.
  • Students are encouraged to register with Disability Services six to twelve months prior to their enrollment at Wright State University.

Apply and Submit Documentation

Submit ALL of the Following:

  1. Application for Services Form: Download PDF
  2. Professional documentation of your disability (please submit all forms applicable to your disability or disabilities):
  3. A list of services provided in high school and/or other schools attended
    • Individual Education Plan (IEP), Evaluation Team Report (ETR), 504 Plan, and/or letter describing services used at previous colleges attended
  4. A transcript of grades from high school and/or other schools attended

All documents submitted are placed in a confidential student file and will not be returned to the applicant,  nor forwarded to any agency or other college or university. We suggest you submit copies, keeping the original documents for your use at a later time.


Schedule a Pre-Service Interview

Once all documentation materials have been received and reviewed, you will be contacted by Disability Services to schedule a pre-service interview.

The pre-service interview is designed to discuss:

  1. services a student is eligible to receive based upon the documentation submitted,
  2. disability-related needs, and
  3. preparations for attending college.

The student and their parents are invited to attend the pre-service interview, which should be scheduled prior to entering Wright State.  This meeting will be held with a professional staff member of Disability Services.

Wright State has adopted a college preparatory curriculum policy. Because our program is supportive in nature rather than remedial, we have found that students with disabilities who are most successful at the university level have taken a college preparatory curriculum in high school. If a student has completed a general high school curriculum, we recommend that consideration be given to entering a two-year college, as a more realistic path to Wright State University.
