The Office of Disability Services (ODS) makes applying for your accommodations as easy as possible. Students with any type of disability can apply for services at any time before or during their enrollment at Wright State (registration will not be finalized until the student is accepted to WSU). Follow the procedure below to apply online today!

Please note: The application for services from the Office of Disability Services is separate from the application for admission to the university, and that ODS does not participate in the university admission process.


Request Services

To request disability accommodations, please submit your request for accommodations via our online application portal*:

Once you submit your online request for accommodations, you will receive an email confirming receipt of your applicationYour confirmation email will also contain a link and instructions to upload documentation of your disability to our online application portal. 


Submit Documentation

Follow the instructions in your application confirmation email to upload documentation of your disability to our application portal.

Students applying for accommodations will submit the following:

1. Professional documentation of your disability

Please submit a certification form for each of your disabilities for which you wish to receive accommodations. The forms below can be completed and submitted by your doctor, or you can provide equivalent documentation.

​2. Individual Education Plan (IEP), Evaluation Team Report (ETR), or 504 Plan from high school (if applicable)

3. A list of services or letter of accommodations provided by other colleges/universities previously attended (if applicable)

Documentation can be submitted at any time during the registration process, and can be accepted in the following formats: 

  • digital file upload to the online application portal
  • email
  • fax
  • in person
  • U.S. Mail


Eligibility Determination

Once all application materials have been submitted and reviewed, ODS will contact you via WSU email and U.S. Mail, to provide instructions for the next step in arranging your accommodations. (This may include the submission of additional documentation.)  

When you receive a letter inviting you to schedule a pre-service interview, you are ready to complete the final step in the application process: to meet with your ODS counselor for an individual interview. During your interview, you will discuss:

  • your eligibility for accommodations/services
  • how to set up services for each term
  • how to inform faculty of your accommodations
  • any questions or concerns you have regarding your accommodations

All documents submitted to ODS are placed in a confidential student file and will not be returned to the applicant, nor forwarded to any agency or other college or university. ODS does not share disability information with any other campus units without prior consent from the student. ODS suggests that students submit copies, keeping the original documents for personal records.
