General/Frequently Used Forms

First Time GI Bill Users at Wright State

Please complete the following two forms once you have registered for classes and the Veteran Enrollment Report for the semester you intend to begin receiving benefits appears below.

Do not complete these forms if the Veteran Enrollment Report for the semester you intend to begin using benefits does not appear, or you will experience benefit delays. Do not complete these forms more than once.

Current Veteran Enrollment Report

All new and continuing GI Bill users must complete the below form to use GI Bill benefits.

Note: If you need to submit a Veterans Enrollment Report for a semester not listed in the form, please email us directly at vmc@wright.edu.

Post 9/11 (Chapter 33) Active Duty/Veteran and Transfer of Entitlement (TOE)

Army Tuition Assistance Users ONLY

Once you have completed your TA submission through the appropriate portal, please send approved TA forms to wsubursar@wright.edu for processing. Please allow 3 business days for processing your TA document. A credit will be placed on your student account for the anticipated amount of your TA. At the end of the term, if your service branch fails to pay for your Tuition Assistance, the credit will be reversed and you will be responsible for all charges. Visit the Student Financial Responsibility Statement website for more information. If you drop classes during the 100% refund period, the TA credit will be removed from your student account.

Uniformed Services Absence Re-Admission Form

 Please complete this form if you have not attended Wright State for more than three semesters and are unable to register for classes. You should complete this form rather than completing the re-admissions process.
