Feeling Down?

For at least the past two weeks, have you experienced:

1. feelings of sadness or depression most of the day for nearly every day?

2. a loss of interest or pleasure in all or most activities each day?
Yes No

3. a significant loss of weight (without dieting) or a significant weight gain?
Yes No

4. a significant decrease or increase in your appetite nearly every day?
Yes No

5. difficulty sleeping or sleeping too much nearly every day?
Yes No

6. feeling restless or feeling dragged down most days?
Yes No

7. feeling fatigued with a loss of energy most days?
Yes No

8. feelings of guilt and/or worthlessness nearly every day?
Yes No

9. difficulties in concentrating or in making decisions?
Yes No

10. recurrent thoughts of death or thoughts of suicide?
Yes No

11. difficulties in meeting your obligations (work, school, relationships) because of your depression?

Please count the number of items for which you answered "YES".  So . . . did you say "YES" to four or more items?    

If you or someone you know has been exhibiting 4 or more of these symptoms, you may want to find someone to talk to about the possibility of depression.

If you or someone you know has frequent thoughts of death or suicide, seek professional help immediately.

Call Counseling and Wellness Services at 775-3407. Walk in intake appoinments are available Monday through Friday from 11 am to 3:30 pm.

If you need to speak to someone after regular business hours you may reach an after-hours counselor by contacting the University Police at 775-2111.

Remember, a high score on this questionnaire does not necessarily mean you have clinical depression--only an evaluation by an experienced clinician can make this determination.
