Health Management Initiative

Overview

Wright State Human Resources is pleased to share our Health Management Initiative (HMI) for the 2021–22 plan year. This voluntary initiative is designed to raise health awareness, increase the detection of cardiovascular disease and diabetes, and encourage participants to begin and/or continue to make healthier lifestyle choices.


New Program Enhancements

The following items are new for this year.

  • A flu vaccine is required (for employees enrolled in a Wright State medical plan as the subscriber). Send proof of your vaccine to HealthWorks by uploading your document to their website.
  • If received an on-campus vaccine from Nursing students, HR will notify HealthWorks.
  • A COVID-19 vaccine is required (for employees enrolled in a Wright State medical plan as the subscriber). Upload an image of your federal COVID-19 vaccination card on Med+Proctor, a secure third-party website.
    • If already submitted in connection with recent university Giveaway, this step is completed.
  • The incentive contribution into a Healthcare Flexible Spending Account (FSA) or Health Savings Account (HSA) has been increased from $400 to $500 provided all requirements are met by March 31, 2022. No incentives are available for spouses or employees enrolled in a Wright State medical plan as dependent spouses.
  • A medical premium discount ($50 estimated monthly savings) beginning January 1, 2023, provided all requirements are met by July 31, 2022.

Based on the aggregate data from 2020, a total of 313 participants received an on-campus biometric screening. Of the participants who received a screening, forty-eight percent were identified as at risk. For the 2021–22 plan year, the initiative will continue to offer the Health Risk Assessment (HRA) and on-campus biometric screenings are required for employees and spouses enrolled on a Wright State medical plan. A Primary Care Physician (PCP) visit is required (for employees enrolled in a Wright State medical plan as the subscriber). To reduce the spread of the flu and COVID-19, both flu and COVID-19 vaccines are required (for employees enrolled in a Wright State medical plan as the subscriber). 

This voluntary initiative requires employees enrolled in a Wright State medical plan (as the subscriber):

  • to complete a confidential online Health Risk Assessment (HRA), 
  • receive and submit verification of the flu,
  • receive and submit verification of the COVID-19 vaccines,
  • receive an on-campus biometric screening,
  • a Primary Care Provider (PCP) visit, and submission of the Physician Verification Form to HealthWorks.

If all requirements are completed before Wednesday, March 31, 2022, a $500 employer contribution will be deposited into a tax-advantaged medical savings account. PPO 80/20 plan participants will receive $500 in a Healthcare Flexible Spending Account (FSA) and HDHP participants will receive $500 in their Health Savings Account (HSA). To receive the $500 deposit, an employee must be actively enrolled in a Wright State medical plan (as the subscriber) on May 31, 2022.  In addition, a medical premium reduction ($50 estimated monthly savings) will be applied beginning January 1, 2023.

If all requirements are completed by July 31, 2022, a medical premium reduction ($50 estimated monthly savings) will be applied beginning January 1, 2023.  Participants are not eligible for the $500 tax-advantaged medical savings account incentive.

Spouses and employees enrolled on a Wright State medical plan as dependent spouses can participate in the program. Participation is voluntary and no incentives are available.


Privacy and HealthWorks' IT Practices and Procedures

HealthWorks, Wright State's previous lifestyle screening vendor, will administer the HRAs, biometric screenings, and collect the PCP Verification Forms. Participants' privacy is strictly protected! The university receives only aggregated data for the participating population and will never know individual HRA answers or biometric screening results.

Health Risk Assessment (HRA)

The HRA is a confidential, self-administered survey that asks a series of questions about your lifestyle habits and health history and only takes 10 minutes to complete. It can be completed 24/7 and is an important first step to improving your health.

Biometric Screenings (Blood Draw)

Biometric screenings provide a clinical assessment of key health measures. These results will identify or indicate an increased risk for health conditions, such as diabetes and heart disease. This will take 10 minutes to complete and is an important part of creating a healthier you and a way to save on your future health care expenses. The HealthWorks website will allow you to schedule your screening 24/7. On-campus screening events will take place at Dayton and Lake Campuses. 

Free biometric screenings include:

  • Blood chemistry profile and complete blood count 
  • 27 individual tests including cholesterol, A1c, glucose, kidney and liver enzymes, and additional tests to detect anemia, infection, and more
  • Blood pressure
  • Body mass index (BMI)
  • Waist circumference 

Screening tips:

  • Results are most accurate with an 8–10 hour fast before your appointment
  • You may drink black coffee or tea (no cream or sugar), and water is encouraged
  • Take all medications as prescribed by your doctor

To begin your journey to a healthier you, complete these 7 easy steps.

Establish a HealthWorks Online Account

  1. Visit www.cincyhealthworks.com
  2. Click on Elevate Login.

New to the HealthWorks portal? The login process requires you to register your email - click ‘register here’ and follow prompts (input your name as it appears on your Anthem insurance ID).  Once you register your email, it becomes your username.

Are you a returning HealthWorks user?  The login process requires you to register your email - click ‘register here’ and follow prompts (input your name as it appears on your Anthem insurance ID).  Once you register your email, it becomes your username.

Schedule an appointment for your on-campus biometric screening online or by phone

  1. After you have completed the registration process, click on the My Appointments tab.
  2. Click on Onsite Screening.
  3. Select a date and time.
  4. To schedule an appointment by phone, call HealthWorks at 513-751-1288.

Complete your HRA

Submission Deadlines:         

March 31, 2022, for the $500 incentive and medical premium differential.
July 31, 2022, for the medical premium discount only.

  1. After you have completed step 2, click on the My Wellness tab.
  2. Click on the Health Risk Assessment.
  3. Complete the assessment.
  4. Be sure to click Save when all sections are completed.

Submit your Federal COVID-19 Vaccination Card

Submission Deadlines:         

March 31, 2022, for the $500 incentive and medical premium differential.
July 31, 2022, for the medical premium discount only.

A COVID-19 vaccine is required (for employees enrolled in a Wright State medical plan as the subscriber). Submit your federal COVID-19 vaccination card on Med+Proctor, a secure third-party website. If already submitted in connection with the recent university Giveaway, this step is completed.

Please note: When you upload an image of your vaccine card you will be given an option to submit using "M+P Pro," which charges a fee for additional services. You do not have to pay a fee to report your vaccination status with Wright State.

You can get vaccinated at Wright State Physicians on the Dayton Campus or at a variety of locations throughout local communities. Learn more at gettheshot.coronavirus.ohio.gov.

Vaccination remains the most effective way to prevent the spread of COVID-19. Getting vaccinated can also help ensure Wright State can continue to offer a full campus experience and protect community members who cannot get vaccinated.

Submit your Flu Vaccine Verification

A flu vaccine is required (for employees enrolled in a Wright State medical plan as the subscriber). Submit your verification to HealthWorks by your provider by uploading it to the HealthWorks website.

View Your Results

Once you have completed the HRA and received your biometric screening, log in to www.cincyhealthworks.com (within 3 to 5 business days of completing your biometric screening) to view your Health Risk Report. The report combines your results from your HRA and biometric screening. In addition, it will provide you with your results and recommendations about making healthier lifestyle choices. You can also view your Biometric Screening Results online. A copy of your Personal Report and your lab work from LabCorp will be mailed to your home address.

Steps to view your results:

  1. Visit www.cincyhealthworks.com
  2. Click on Elevate Login.
  3. Enter your username and password
  4. Click on the My Wellness tab.
  5. Click on Biometric Screening Results and review your report.
  6. Click on Health Risk Report and review your report.

Primary Care Physician (PCP) Visit

Submission Deadlines:         

March 31, 2022, for the $500 incentive and medical premium discount.
July 31, 2022, for the medical premium discount only.

Once you have obtained your screening results, schedule an annual routine visit with your PCP. Take your screening results and the HealthWorks’ Annual Physician Verification Form (PDF) to your appointment to review your screening results and discuss your overall health and applicable preventive cancer screenings. The Annual Physician Verification form must be signed and submitted to HealthWorks via one of the following methods:

Additional information regarding PCP Visits

  • Each calendar year, Wright State's medical plans cover one annual routine visit with a PCP at no cost to you.
  • If you received an annual visit in 2021 you should consider scheduling your appointment after January 1, 2022, to avoid any out-of-pocket expenses.
