Health Management Initiative

Wright State Human Resources is pleased to announce a new Health Management Initiative (HMI).  This voluntary initiative is designed to raise health awareness, increase detection of cardiovascular disease and diabetes, and encourage employees to begin and/or continue to make healthier lifestyle choices.

This new HMI is open to all employees enrolled in a Wright State medical plan (as the subscriber) and requires you to complete both a confidential online Health Risk Assessment (HRA) and on-campus biometric screening. If you complete both before Wednesday, October 23, 2019, you will receive a $300 employer contribution to either a Healthcare Flexible Spending Account (FSA) or Health Savings Account (HSA), if employed and enrolled in a Wright State medical plan (as the policyholder) on January 31, 2020. HealthWorks, Wright State’s previous lifestyle screening vendor, will administer the HRAs and biometric screenings.

Health Risk Assessment (HRA)

The HRA is a confidential, self-administered survey that asks a series of questions about your lifestyle habits and health history and only takes 10 minutes to complete. It can be completed 24 hours/7 days a week and is an important first step to improve your health.

Biometric Screening (Blood Draw)

Biometric screenings provide a clinical assessment of key health measures. These results will identify or indicate an increased risk for health conditions, such as diabetes and heart disease.  This will take 10 minutes to complete and is an important part of creating a healthier you and a way to save on your future health care expenses.  The HealthWorks website will allow you to schedule your screening 24 hours/7days a week.  On-campus screening events will take place at Dayton, Lake and STEM School campuses.

Free biometric screenings include:

•     Blood chemistry profile and complete blood count

- 27 Individual tests including cholesterol, A1c, glucose, kidney and liver enzymes, and additional tests to detect anemia, infection and more.

•     Blood pressure

  • Body Mass Index (BMI)
  • Waist circumference

Screening Tips:

  • Results are most accurate with an 8-10 hour fast before your appointment
  • You may drink black coffee or tea (no cream or sugar), and water is encouraged
  • Take all medications as prescribed by your doctor

To begin your journey to a healthier you, complete these 4 easy steps. 


To establish a HealthWorks online account

  • Visit
  • Click on Elevate Login:
    • For security purposes:
      • All passwords from last year have been reset
      • Although the HealthWorks website indicates “Last 4 digits of your social security #”, this is incorrect for Wright State accounts. Please use the last 4 digits of your Wright State UID# instead.
  • Username:  Enter the first initial of your first name + first 4 letters of your last name + the last 4 digits of your WSU UID#
    • Example:
      • Name: Rowdy Raider
      • Wright State UID#: U00012345
      • Username: RRAID2345
  • Your temporary password is APPLE (all caps)
  • Create and verify your own password
  • Complete the registration process

STEP 2: (Deadline: October 23, 2019)

To schedule an appointment for your on-campus biometric screening via online or by phone

  • After you have completed the registration process
  • Click on the My Appointments tab
  • Click on Onsite Screening
  • Select a date and time

To complete your HRA (Deadline: October 23, 2019)

  • After you have completed step 2
  • Click on the My Wellness tab
  • Click on the Health Risk Assessment
  • Be sure to Save when all sections are completed


View Your Results

Once you have completed the HRA and received your biometric screening, log in to (within 3-5 business days of completing your biometric screening) to view your Health Risk Report.  The report combines your results from your HRA and biometric screening.  In addition, it will provide you with your results and recommendations about making healthier lifestyle choices. You can also view your Biometric Screening Results online. A copy of your Personal Report and your lab work from LabCorp will be mailed to your home address.

  • Visit
  • Click on Elevate Login
  • Username: First initial of your first name + first 4 letters of your last name + the last 4 digits of your WSU UID#
  • Enter your password (established during your account set-up)
  • Click on the My Wellness tab
  • Click on Biometric Screening Results and review your report
  • Click on Health Risk Report and review your report
