Incentive-Based Program

Pursuant to the elimination of the living WELL Employee Wellness Program, please read this important information regarding the employee wellness incentive:

  • Wellness cash payout for participating Staff and Non-Bargaining Unit Faculty Members:
    • The payout originally scheduled for the end of December will be moved up to the end of September. 
    • The new deadline for completing required actions and submitting completed health actions into the wellness portal is September 11, 2018.  See Participation Requirements in the table below.
      • Important note on portal workshops: Some wellness portal workshops, including the required Personal Goal Setting workshop, are delivered over multiple weeks (3 or 2 weeks).  Workshops need to be started and completed by September 11, 2018 in order for them to count toward this incentive.
  • Dayton Campus Fitness Center annual fee waiver for 2018 for participating Staff and Non-Bargaining Unit Faculty Members:

Living WELL is an incentive-based employee wellness program that runs annually and is available to all benefit-eligible (51% + FTE) Wright State University employees.  The 2018 living WELL program year runs January 3 through September 11, 2018.  Employees must enroll (or re-enroll, if they participated in previous years) in the program at the start of each program year.

There are different incentives and participation requirements to earn the incentives for the following two employee groups:

  1. Staff and Non-Bargaining Unit Faculty Members (Non-BUFM)
  2. TET and NET Bargaining Unit Faculty Members (BUFM)

Please reference tables below.

Participation in the program includes

  • Enrolling on the wellness portal, then completing a personal health assessment and biometric screening to help gain awareness and understanding of your health status, so that you can create goals and take actions toward your optimal health and well-being. (Applies to Staff, Non-BUFM, & BUFM)
  • Engaging in health actions and tracking completion of them in the wellness portal during the program year.  Examples of health actions include physical activity and healthy eating tracking,  utilization of preventive health care and screenings, and completion of online health promotion workshops. (Applies to Staff & Non-BUFM)

Participation in the program is 100% voluntary. However, eligible employees who choose to participate can earn incentives for completing health actions.


Incentives and Participation Requirements

For Benefit-Eligible Staff and Non-Bargaining Unit Faculty Members (non-BUFM)
Incentives Required actions to earn incentive
Waiver of the Dayton Campus Fitness Center Annual Membership Fee
  1. Create a personal account on or re-log on to the wellness portal by September 30, 2018.
  2. Complete the Personal Health Assessment on the wellness portal by September 30, 2018.
  3. Complete and submit a biometric screening by September 30, 2018*
  • *A biometric screening completed 7/1/2017 – 12/31/2017 by a personal health care provider can be submitted to apply to 2018 program year requirement.
  • *Biometric requirement is waived for new employees with a hire date of 7/1/2018 or thereafter in 2018.

  • (Fee waiver will be applicable after the required actions are completed in 2018 and participation verification is administratively processed.)
Up to $130 year-end cash payout In order to qualify for any amount of year-end cash payout, the following 5 steps (worth 500 points -- $50) need to be completed by September 11, 2018.
  1. Create account/re-log on to wellness portal
  2. Complete PHA (150 points)
  3. Complete Biometric Screening (200 points)
  4. Complete the Personal Goal Setting Portal Workshop on the wellness portal (50 points)
  5. Complete 2 other Portal Workshops of your choosing on the wellness portal (100 points)

Earn up to an additional 800 points -- $80

  • Earn up to 800 more points (maximum 1300 points total) by completing a variety of other health actions in 2018 unitl 9/11/2018
  • Each point equals $0.10
  • Cash payout will be distributed on the last paycheck of September 2018
For Benefit-Eligible TET and NET Bargaining Unit Faculty Members (BUFM)

How to Enter Health Actions and Check Incentive Attainment in the Wellness Portal

  • When creating a new account, do not use your actual month and day of birth but instead use “1/01” for month/day and your actual birth year.  Example: if your DOB is April 5, 1955, then enter 1/01/1955.  See “Getting Started” for further instruction on how to enroll.
  • Incentive Points Summary: To view a list of health actions and their related point values, on the wellness portal from your Dashboard (home page) click on the "2018 - List of point values and yearly maximums" link under "Quick Links."
  • Portal Trackers:  Once logged on, the following health actions can be entered via the portal trackers in the appropriate horizontal menu sections:

  • Fitness Devices and Applications: The wellness portal is integrated with many wearable fitness devices and applications, such as Fitbit, Garmin Connect, and more.  Once your device or app is synched with the portal, cardio activity can be recorded in your portal from your device.  To set up your device or app, from your Dashboard page click on the Connect Devices & Apps and follow the instructions on the Applications & Devices page.
  • Other Health Actions (Healthy Habits): Other completed health actions can be entered under the Healthy Habits page.  From the horizontal menu on your Dashboard, click Incentives > Healthy Habits
    • Check the box to the left of all completed items you wish to submit, and fill in the appropriate date in the text box.   
    • Scroll to the bottom of the list and click the green “Update” button.  **If this step is skipped, items will not be loaded and thus no points awarded.  
  • Portal Workshops: Portal workshops can be accessed from the Dashboard by clicking the Portal Workshops button at the top (“wellness workshops” green apple image).
    • Select the workshop(s) in the “Add” section and click “Signup” to add to your “Active” and “Queue” sections.
    • When a portal workshop is complete, the portal will automatically award points.  Some workshops are delivered over more than one week.

Checking Incentive Attainment

  • Fitness Center Annual Membership Fee Waiver
    • Once you have completed the PHA and Biometric Screening, and the data has been securely loaded into the wellness portal, you should see two green bars for those respective health actions on your wellness portal Dashboard page “Incentives” widget.  This indicates that you have qualified for the Fitness Center annual membership fee waiver.
  • Year-end Cash Payout
    • Once you have completed the PHA, Biometrics, Personal Goal Setting Workshop and 2 other Workshops of your choosing, and the data has been securely loaded into the wellness portal, you should see full green bars for the first three activities (PHA, 150 points; Biometric, 200 points; Personal Goal Setting Portal Workshop, 50 points) and at least a half green bar (100 points) for the fourth activity ("Complete a minimum of 2 Other Portal Workshops").  This indicates that you have qualified for the year-end cash payout. (Biometric is waived for new employees hired 7/1 or thereafter).
    • To see your total points earned in your wellness portal, from the horizontal menu on your Dashboard go to Incentives > Incentive Summary and scroll to the bottom of the page.
    • Each point equals $0.10.  A maximum of 1300 points ($130) can be earned in a program year.

Fitness Center Annual Membership Fee Waiver - Incentives Widget Example

Once you have completed the PHA and Biometric Screening, and the data has been securely loaded into the wellness portal, you should see two green bars for those respective health actions on your wellness portal Dashboard page “Incentives” widget.  This indicates that you have qualified for the Fitness Center annual membership fee waiver.

To receive the fitness center waiver, you need to complete the Personal Health Assessment and Biometric Screening.

Year-end Cash Payout - Incentives Widget example
