Maintenance Request Form

**If your concern is about maintenance billing or an appeal of damage charges, please view our separate "Maintenance Appeals" section.**

If this is an EMERGENCY request, such as no heat, no water, no electricity, or plumbing problems:
During business hours (8:30 a.m. – 4:30 p.m.), call us at:

  • (937) 775-4239 for Forest Lane and Hamilton Hall
  • (937) 775-4141 for the Village, College Park, University Park, Honors, and the Woods.

After hour emergency requests should be called into the WSU Police Department Dispatch at (937) 775-2111 and they will contact the Residence Life staff or Maintenance staff on call.
