About Assessment at Counseling and Wellness Services

Counseling and Wellness Services (CWS) offers several types of assessment as a standalone service, meaning that the individual may participate in the assessment without being involved in any other services at CWS. The types of assessment offered as a standalone service are assessment for Attention-Deficit/Hyperactivity Disorder, assessment for learning disorders, and assessment for diagnostic clarification.

Assessment services are provided at no additional cost if the student pays the semester CWS Optional Fee. If the CWS Optional Fee has not been paid, the cost is $20 per hour of assessment. A $20 missed session fee will be assessed under the following conditions:

  1. The client does not show for their appointment
  2. The client provides less than 24 hours notice that they will miss an appointment
  3. The client is more than 15 minutes late for the appointment without prior arrangement

If the client misses or is more than 15 minutes late for an initial assessment appointment without providing advance notice, they will no longer be eligible to participate in a standalone assessment. Exceptions to this policy based on extenuating circumstances may be made at the discretion of the Director, Counseling and Wellness Services.

Attention-Deficit/Hyperactivity Disorder Assessment

This type of assessment is recommended when a person is experiencing persistent difficulty with attention and/or impulsivity across multiple settings such as during class, at work, and at home studying or completing household chores. The assessment involves participation in a one-hour interview, approximately three hours of testing that will be spread out over two appointments, and a one-hour feedback session.

Learning Disorder Assessment

Learning disorder assessment is recommended when a person is having difficulty in a particular academic area, such as reading or mathematics. The assessment involves participation in a one-hour interview, approximately four hours of testing that will be spread out over two appointments, and a one-hour feedback session.

Assessment for Diagnostic Clarification

Diagnostic Clarification Assessment is offered to provide information to an individual about their emotional functioning, how they respond to and manage stress, how they approach relationships, and how they see themselves. This type of assessment may or may not result in the diagnosis of a mental health disorder. The assessment involves participation in a one-hour interview, approximately two hours of testing, and a one-hour feedback session.
 

What to Bring to Your Assessment Appointment

For any type of assessment, bring the following to the initial appointment:

  • Records of any previous assessment, if available
  • Records of any previous mental health treatment, if available
  • Contact information for current and previous assessment and mental health treatment providers

For an Attention-Deficit/Hyperactivity Disorder or learning disorder assessment, bring the following in addition to items listed above:

  • Educational records, if available (include report cards and any individualized education plans)
  • Contact information for all schools attended
     

What to Expect at Your First Assessment Appointment

Your first appointment will be one hour. You will meet with your assessment provider, who will:

  • Ask questions about the concerns that led to your participation in an assessment
  • Gather information about your personal background and history
  • Ask you to provide copies of any records that you were able to locate
  • Ask you to complete releases of information so that records of assessment and treatment, educational records, and information from family members can be obtained if necessary.
     

What to Expect at Your Feedback Session

You can expect to be scheduled for a one-hour feedback session within approximately one month of your final testing session. At the feedback session you will:

  • Review a written report of the assessment results with your provider
  • Have an opportunity to ask questions about the testing process and results
  • Request that a copy of the report is sent to another provider, another office on campus, etc.
