Below you can find all of the forms you need to request service or support for technology at Wright State. If you can't find the form you're looking form, contact the CaTS Help Desk for further assistance.

  • Account Application (PDF)
    Fill out a request for a computer account for new staff/faculty, along with special access needed. Requests for long term guest accounts and delegated email accounts.
  • Account Name/Email Change (PDF)
    Request to change campus username account, and/or email address. Note: You must have your information updated with H.R. (faculty/staff), or the registrar (students) first.
  • Account Verification Form (PDF)
    Request access to your personal account information if the Help Desk is unable to verify you and you cannot appear in person.
  • Alumni Account Application (PDF)
    For graduates prior to 2006 to request an Alumni email account. After 2006 access is automatic.
  • Alumni Account- Request to Close (PDF)
    As a graduate of Wright State with access to your Alumni email account, fill out a request to have your account closed.
  • AnyConnect VPN Registration
    The AnyConnect VPN from Cisco allows authorized users to securely connect to the WSU data network from the campus wireless network or from off campus using various commercial Internet Service Providers (ISPs) (e.g., MSN, AOL, Roadrunner, SBC, etc.).
  • Appearance Release (PDF)
    Grant Wright State permission to use your name and/or one or more portraits, pictures, photographs, video and audio recordings of you ("Appearances"), or reproductions or derivatives of the same, in any form of education, communication, and/or promotion purposes, unless otherwise noted.
  • Banner Admin Access (PDF)
    Fill out a request to access various restricted modules in Banner (STUDENT INFO, Financial Data, etc). Applicants will need authorization from their Department Manager and the departmental approver for data being accessed.
  • Bulletin Board Announcement Request (PDF)
    Submit a request for a University related event to be featured on Wright State’s Cable Bulletin Board (WSU Channel 21).
  • Classroom Equipment Preferences Form
    As a Faculty/Staff member, submit a request for your preferred equipment to be accessible from a specific classroom (must be submitted 8 weeks before the classroom will be used).
  • Classroom Problems and Requests
    Use this form to report problems to an electronic or computer classroom. The information on this form will be reported to the appropriate department via email. If the problem you are having requires IMMEDIATE assistance, please do not use this form. Instead, contact the CaTS Help Desk at (937) 775-4827 for assistance.
  • COGNOS Access Request Form (PDF)
    This application is for Wright State University staff and faculty with an educational interest and need to access information from one or more of the module areas listed. Applicants will need authorization from their Dept. Chair, College Dean, or Dept. Director.
  • Comments/Suggestions
    Submit a question, comment or concern and your information will be logged in CaTS problem tracking system. You will receive a response in the form of an email within the same day.
  • Equipment Request Form (PDF):
    Request a specific piece of equipment to be used for educational purposes. (Ex. digital camera, wireless microphone, etc.).
  • Exception Request
    Request an exception from a current CaTS policy in order to accommodate a technological need or request that falls outside of the policies set forth by CaTS.
  • Faculty Listserv Subscriptions
    Use this page to subscribe and unsubscribe from the university email lists.
  • Faculty Backup Drive Request (Faculty Members Only)
    All current faculty members, deans, and chairs may request an external USB hard drive for the primary purpose of backing up their university-owned computer system.
  • Guest Account Requests
    Short term guest account for visitors to Wright State needing internet access sponsored by a department. Form requires authentication by the department sponsor. Maximum 30 days access.
  • Media Duplication Request (PDF)
    Submit a request for a University college, department or event project to be duplicated. You must provide a valid FOPAL for billing charges.
  • Network Shared Area Request (PDF)
    Used to grant access to a department shared area or restricted area.
  • Non-Supported Product Form (PDF)
    If you prefer to purchase a product not supported by CaTS, fill out this form acknowledging your understanding of the responsibilities that come along with using this non-supported product (more information through link).
  • Non-WSU User Accounts for CaTS Computer Facilities (PDF)
    Accounts for Non-WSU individuals attending a Wright State sponsored workshop in a CaTS computer lab.
  • Server Registration
    Use the link to learn how to register a new or existing server on the WSU network. You can also read information about what happens after you submit your server registration form, and how CaTS will follow up with you concerning the registration.
  • STEM School Student Computer Account Application (PDF)
    As a Dayton Regional STEM School Student, submit this form for a Wright State University computer account.

  • System Modification Request
    (requires authorization with campus username and password)
  • Telecommunications Service Request (PDF) The Telecommunications Service Request is a form that is submitted to Telecom for any billable work that needs done.
  • TruAware Training Request Form
    Fill out this form to request access to specific training that is available through TruAware. Supervisors or teachers should include names and email addresses of any trainees that will be participating in the training.

    Submit a request for access to either the Finance and/or WrightBuy programs through WINGS Express.

    Accounts for Non-WSU individuals attending a Wright State sponsored workshop in a CaTS computer lab.
  • WSU Web Server Access Request
    Any university-affiliated department, program area or organization may request access to a university server for the purpose of publishing information about the services they offer.
