Microsoft Forms is a new part of Office 365 Education that allows you to quickly and easily create custom quizzes, surveys, questionnaires, registrations, and more. This service is accessible to all students, faculty, and staff.
Note: Microsoft Forms is currently in 'Preview' mode; the look and functionality may change over time.

Benefits of Microsoft Forms
When you create a form, you can invite others to respond to it using any web browser or mobile device. As results are submitted, you can use built-in analytics to evaluate responses. Form data can also be easily exported to Excel for additional analysis or grading.

Usage Instructions
To begin using Forms, sign in to your Wright State Office 365 account and open the Forms application in the menu bar.
Note: If you do not see the Forms icon in the application grid, click the 'View all my apps' link at the bottom.
The Forms icon will look like the image below before it is set up in your account:
Click the tile to begin set up; you will not notice any changes as the application is setting up. Once set up is complete, the Forms icon will appear like the image below:
How to Access Microsoft Forms
You can access Forms using the following methods:
WINGS Website
- Log in to WINGS with your "w" username and password
- Click on the Office 365/Email link in the top right corner to access your Office 365 account
- Click on the grid in the top left corner of the Office 365 screen
- Select the Forms application
Office 365 Login Page
- Log in to Office 365 (http://outlook.wright.edu) with your Office 365 username and password
- Click on the grid in the top left corner of the Office 365 screen
- Select the Forms application
