The period covered by the annual performance evaluation forms this year is April 1, 2016 to March 31, 2017. The timeline for this year’s annual submission is:

  • March 31, 2017 – Performance evaluation period closes
  • April 7, 2017 – Self-Appraisal Form to be submitted to supervisor
  • April 28, 2017 – Formal Performance Evaluation Form to be signed by employee
  • May 5, 2017 – Signed Formal Performance Evaluation Form to be delivered to Human Resources
  • June 1, 2017 – Human Resources to report supervisor compliance with process to Deans/VPs
