The STD benefit, provided by Unum, protects employees against the loss of income for the first 26 weeks of a qualifying disability. This coverage will pay a benefit equal to 60% of an employee’s covered WSU earnings at the time of disability (maximum benefit, $2,310 per week). This benefit may be reduced by other income benefits. Before collecting STD benefits, an employee must satisfy the elimination period following the date of disability. This period ends on the later of the date the employee’s sick leave is exhausted or 14 calendar days of continuous disability.
If you decided not to enroll upon your initial employment, the next opportunity is annually during Open Enrollment. If you currently have Short Term Disability Coverage and wish to continue – you do nothing.
Full time employees (75% FTE or higher) are eligible to apply for coverage.
Evidence of Insurability is Required:
Evidence of Insurability is required and includes a review of your overall medical health including routine, planned, unplanned or ongoing medical care or consultation, and may result in a declination of coverage.
Cost of Coverage:
The cost for this insurance is paid by the employee with payroll deductions and can be found on the Short Term Disability Enrollment Form.
To Apply During Open Enrollment:
You will need to complete two forms during the Open Enrollment period:
- Short Term Disability Enrollment Form (PDF) – forward completed form to Jamie Henne, Leave and Disability Coordinator in Human Resources; 115 Medical Sciences Building.
- Short Term Disability Evidence of Insurability (PDF) – mail directly to Unum at the address located on the first page.
