|Drug – Max of 30 Days’ Supply||Copayment at Time of Payment|
|Drug – Greater Than 31 Day Supply (must use mail order)||Copayment at Time of Payment|
|Tier 2||10% of cost (minimum of $22)|
|Tier 3||10% of cost (minimum of $60)|
High Deductible Health Plan Election
Employees who elect the High Deductible Health Plan (HDHP) pay Anthem’s discounted drug cost until the plan’s Network Deductible and Out‐of‐Pocket Annual Maximums have been satisfied. Then, the plan pays 100% benefits for all covered network expenses, including prescription drug costs, for the rest of the calendar year. Up to a 90-day supply of maintenance drugs are available through Anthem’s mail service pharmacy, Express Scripts.
