Q:  How do I make changes to my insurance coverages?
A:  You can make changes to insurance coverages by completing the appropriate enrollment form during open enrollment or for healthcare, at any time with a qualifying change to family status.

Q:  Who is considered an eligible dependent for health insurance purposes?
A:  Eligible dependents for health insurance purposes are spouses, income tax dependent children, registered domestic partners, and the income tax dependent children of registered domestic partners.

Q:  How do I get reimbursements from my flexible spending account?

A:  First, you must file a claim with the medical, dental or vision vendor.  Once you receive an Explanation of Benefits (EOB) or a receipt (office visit or prescription drug copayment) for the cost of services reflecting your actual out of pocket expenses, complete a Flexible Spending Account (FSA) Request for Reimbursement form.  Attach the EOB or receipt to the FSA Request for Reimbursement form and mail the documentation to:

Q:  Why isn’t my Anthem card accepted by my dentist?
A:  Your Anthem ID card is not accepted by your dentist because dental benefits are provided by the dental plan provider, not the medical plan provider.

Q:  If I change from full-time to part-time, can I still get insurance benefits?
A:  Employees who are appointed to work at least 51% FTE are eligible for healthcare coverages and life insurance coverages.  An employee must be at least 75% FTE to be eligible for Long and Short Term Disability.

Q:  Will our insurance cover my child who is out of state attending college?
A:  Yes, healthcare benefits cover eligible dependent children who are out of state attending college.  Medical benefits are available dependent upon your choice of health plans.  Vision and Dental benefits are also payable depending on the provider from which you receive care.
