Health, Dental, and Vision Benefits
Guidelines
Effective November 1, 2002 Wright State University has designated
Advanced Benefit Planning, Inc. (ABP) as our exclusive partner for
medical, dental and vision insurance benefits consultation. Our
partnership with Advanced Benefit Planning, Inc. is administered
by the Department of Human Resources.
It is our intention in contracting with Anthem, Delta Dental and
Vision Service Plan plus partnering with ABP that you receive the
full value of the benefits to which you are entitled. For your convenience
in getting answers to your medical, dental and vision benefit questions,
the links below include carrier information, a benefit inquiry form,
a medical release form and answers to questions that you may have.
Step 1 The first step in getting answers to your
questions is a telephone call or website contact with the carriers.
We expect that most of your questions will be answered through the
carrier’s customer service department. Go to the link below
for carrier contact information.
Step 2 The second step, should your inquiry need
a higher level of expertise, is to contact Advanced Benefit Planning,
Inc. (ABP). ABP may be contacted by telephone or by completing and
sending by e-mail, by fax or by mail a Benefit Inquiry Form. Go
to the Benefit Inquiry Form link below.
If your inquiry in any way requires the discussion by ABP with
a doctor’s office, hospital, lab, etc. regarding a medical
service provided or clarification of a treatment rendered, it will
be necessary for you to complete a Medical Release Form. Go to the
ABP Medical Release Form link below to print a Medical Release Form.
Completing the form will require your signature. ABP’s fax

The Questions and Answers section should
provide answers to many of your initial questions. For clarification

Please remember the terms of Wright State’s partnership
with ABP require that you contact the carrier first and then ABP.

life insurance and long-term disability insurance benefits as well
as with our retirement plans.
