Outstanding Teaching Award Nomination Form : Faculty

The College of Science and Mathematics each year presents a cash award and certificate to two or three of its faculty in recognition of outstanding teaching. An Awards Committee is now accepting nominations for awards for this academic year. Any student, colleague, or alumnus is invited to submit a nomination by providing information on the form below. All full-time faculty (*except winners of teaching awards for the last four years) are eligible for nomination. Please consider this opportunity carefully.
