Go to www.wright.edu
Click on “R”
Click on Registrar
Click on Forms (on the left side near the bottom)
Under Privacy Forms, choose:
Student Authorization for Release of Information - or -
Instructor Letter of Recommendation
Print the form and give to the instructor
