This form is for reporting problems or questions to the Help Desk via the WWW. It is not to be used for requesting accounts or passwords. To request an account or a password reset, take a picture ID to the Help Desk in 025 Library Annex.

Please provide a detailed description of your problem/question and the appropriate contact information. Problem submissions are automatically logged to our tracking system and receive equal treatment. If you require an immediate response, please call the Help Desk.

If you do not receive an e-mail with a HEAT reference number for your problem within a day, please call the Help Desk at 775-4827 and report your problem.
