Forms were created in Adobe® portable document format. Some forms can be completed onscreen then and printed for submission to Student Employment. Some forms must be completed manually. Forms can be downloaded using Adobe® Acrobat Reader®.

Student Employment Hiring Packets

After you are offered an on-campus, part-time Student Employment position, your Student Employment Hiring Packet must be completed, submitted to Student Employment and processed before you begin working. - Complete the Student Employment Hiring Packet that matches your Student Employment situation. Some forms require your supervisor's input and signature.
- Bring your completed Student Employment forms to Career Services during Student Employment Processing Hours. You must be present while your forms are processed. Student Employment Processing hours are Mondays, Wednesdays, and Thursdays, between the hours of 8:30 a.m. to 11:30 a.m. and 1:00 p.m. to 3:30 p.m.

Forms for First Time Workers
A student who has not worked previously in a student employment position must complete all of the forms listed in this section and submit to Student Employment before he or she begins working.
Completed forms are submitted to Student Employment in Career Services, during Student Employment Processing Hours.
Both the student employee and the supervisor must complete the Student Employment Authorization form and the Student Employment Contract.
The authorized university representative who completes section II of the I-9 form must see original qualifying documents and include a photocopy of the original documents. Be prepared to present your original documentation.

Forms for International Students Working for the First Time
An international student who has not previously worked in a student employment position must complete all forms listed in this section and submit to Student Employment before he or she begins working.
Completed forms are submitted to Student Employment in Career Services, during Student Employment Processing Hours.
Both the student employee and the supervisor must complete the Student Employment Authorization form and Student Employment Contract.
An international student must also have a Social Security Card or receipt of application for the Social Security Card, and a copy of their email from University Center for International Education verifying their Work Authorization.
An international student must also bring his or her Visa and original Passport, form I-94, and form I-20 to Student Employment Processing.

Forms for Students Previously Employed in a Student Employment Job
A student that has worked previously in any student employment position must complete and submit only the forms listed in this section, before the student begins working.
Completed forms are submitted to Student Employment in Career Services, during Student Employment Processing Hours.
Both the student employee and the supervisor must complete the Student Employment Authorization form.
Additional Documents for International Students
An international student who has not worked for an entire quarter in a student employment position must also submit to Student Employment a copy of the email, from University Center for International Education, verifying their Work Authorization.
