* Vietnam-era veterans (Chapter 30/34 conversion) are able to claim dependents. In order to claim dependents you will need to complete a Declaration of Status of Dependents (VA Form 21-686c) and provide the Wright State Office of Veteran Affairs with a certified copy of your marriage certificate and birth certificates of children.

If you have received education benefits at another College/University/Training Program, you will need to complete the following forms:

If you are currently receiving benefits at Wright State University, you only need to complete a Veterans Enrollment Report each quarter.

* You must inform the Office of Veteran Affairs if you drop or add classes, withdraw from school or if you change your major.
