Your VA File Number will be needed for the application. This number can be found on VA correspondence.

* If you have not received a file number and eligibility information from the VA, please contact the VA Regional Office for information at 1-800-827-1000.

If you have received benefits at another College/University/Training Program, you will need to complete the following forms:

If you are currently receiving education benefits at Wright State University, you only need to complete a Veterans Enrollment Report each quarter.

* You must inform the Office of Veteran Affairs if you drop or add classes, withdraw from school or if you change your major.
