Right To Access Exposure and Medical Records:

Letter to all Employees

Medical Surveillance Program:

Various faculty and staff are required to be included in our occupational safety and health medical surveillance program.  If you fall into any of the listed occupations below, you should contact us for further discussion:

  • Athletic Trainers
  • Biosafety, Level 3
  • Camp Counselors
  • EH&S Personnel
  • Gross Anatomy Instructors and Mortuary Personnel
  • Lab Animal Resources Personnel, (or anyone working with animals on campus)
  • Laser Operators
  • Life Guards
  • Physical Plant, (except office personnel)
  • Police Officers
  • Radioactive Material Users
  • Selected Lab Personnel

...and others as deemed necessary. If you feel you need an assessement, please fill out the Medical Surviellance Questionnaire and return to us.
