letter to the Director
of the BMS PhD Program. In it, give DETAILS

Sign
and Date the form in the space provided.
Mail
the Graduate Studies Application form, with
the Statement of Objectives letter attached,
to the School of Graduate Studies.

Send transcripts
from all the colleges and universities you
have attended to the School of Graduate Studies.
The
address is on the Application for Admission
to a Graduate Program form (above).
Download a BMS
PhD Program Recommendation Form, or request
one from the BMS PhD Program Director.
Three
persons acquainted with your work must submit
a BMS Program Recommendation Form and a letter
on your behalf. The letter should explain
how the person knows you and evaluate your
readiness for graduate school (quality of
work, maturity and motivation). The
person making the recommendation should mail
it. It will delay or cancel consideration
for admission if you mail it instead.
Make three copies of the form, and complete
the Applicant Information at the top of
EACH form.
Give a form, already signed by you, to EACH
person recommending you.
Provide an envelope addressed to the Biomedical
Sciences PhD Program. The address is on
the form.
Have
the recommendation forms sent directly to
the Director of the BMS PhD Program.
Do
not send them yourself. The
address is shown below.
Financial
aid. No application for financial support
is required. All successful applicants are
considered for BMS assistantships and fellowships.

Submit
all materials by March 1
ALL APPLICANTS (including International
Students): Mail your application

REFERENCES: Three letters
